Hospital at Home (H@H) referrals

H@H provides an alternative to acute hospital admission, treating and monitoring patients so that they can remain at home or in their care home during an acute illness.

We also facilitate early discharge from hospital, providing care that would previously been provided in an acute setting.

Our aim is to provide treatment or care in the patient's home that is equivalent to inpatient care.

Patients cannot self-refer for our services.
