If you have a new continuous cough and/or high temperaturedo not come to our hospitals.

Follow the national advice and stay at home for seven days. For most people, coronavirus (COVID-19) will be a mild infection, but for some of our patients it is a higher risk. Please help us to help you.

Important information about non-urgent inpatient operations and outpatient appointments.

From Monday 23 March we will not allow any visitors to our hospitals – with a few exceptions.

The forms at the links below are Word documents that allow certain fields to be filled in; address and order amounts. Please save them on to your local device, including the name of your practice in the new file names.

You may also print out the forms, complete them by hand and email us scans of the completed forms.

Please complete the forms (GP form 3)remembering to enter your address, contact number and transfer point for those who use the two form (GP form 1 and 2) system.
