Home Assessment Reablement Team (HART)

The Home Assessment Reablement Team - known as 'HART' - is a service commissioned by Oxfordshire County Council within Oxford University Hospitals.

HART Home Assessment Reablement Team

We provide a short period of care and support (no longer than six weeks) to people in Oxfordshire who need help to regain the independence, confidence and skills to live safely at home.

HART offers 'reablement' support both to people considered medically fit enough to be discharged from hospital, and those referred in the community, for example, by their GP or a community hospital.

We work with these people, their families and informal carers, to agree a plan for the support that we will provide.

An assigned Case Manager reviews each person's plan, and a HART Reablement Support Worker visits daily to help them achieve their goals.

If someone needs longer term support, we discuss this with them and the Adult Social Care Team from Oxfordshire County Council, who will explain the costs involved and help them complete a financial assessment.
