Community

First new patient visit - day after discharge from hospital

All mothers and babies who are discharged from hospital, MLU or home birth within 72 hours of birth will receive a face to face appointment on day one of being discharged from the hospital (the next day after discharge).

Mothers and babies discharged from hospital, MLU or home birth 72 hours or more after birth will receive a telephone consultation.

If during the consultation the midwife decides a face to face appointment is required, this will be actioned without delay.

Day five

  • Face to face appointment with the community team
  • Newborn Bloodspot Screening
  • Baby's weight is checked
  • Full postnatal check for mother and baby

Day ten

  • Face to face appointment with the community team
  • Discharge from Maternity Services (around day 10-14)
  • Baby's weight is checked (if required)
  • Full postnatal check for mother and baby
