The Heart Failure Inpatient Nurse Specialists liaise closely with the Oxfordshire Community Heart Failure Service.
Ideally patients admitted to hospital will be seen before discharge by the Heart Failure Nurse Specialists. Written and verbal information is given regarding signs and symptoms, lifestyle advice and when to seek help.
If the patient is not seen before discharge, relevant information is sent to the community heart failure nurses in Oxfordshire and surrounding counties.

Oxfordshire Community Heart Failure Nurse Service works in conjunction with the heart failure service at the John Radcliffe and Horton General hospitals to provide support for the management of heart failure patients in the community.
