Using OUH as a referral laboratory

When OUH Microbiology is used as a reference laboratory (either within the South 4 Pathology Partnership Network or externally), in order to fulfil the requirements of ISO15189:2022 we have made all relevant information available on this website.

Costings are commercially sensitive and can be provided on request from the laboratory manager. The OUH (referral laboratory) and the sender enter into a contract when the appropriate sample and accompanying request form are sent. A separate SLA is not required in these cases.

Consent

It is the responsibility of the requesting doctor to obtain appropriate informed consent for all investigations, including testing for blood-borne viruses.

Identifiers

It is essential that requests, forms, or samples are completed correctly with the minimum of three unique patient identifiers.

  • Unique identification number e.g., hospital number or NHS number
  • Surname / forename
  • Date of birth

Samples received that do not have these three identifiers will not be processed.

Single or two identifiers may be accepted as long as there is a unique identifier number:

  • GUM clinic numbers
  • clinical trial patients
  • chlamydia screening programme.
