Referral process 

Please refer all patients aged 24 years and under for a discussion at the end of treatment at the TYA MDT (Mondays at 1.00pm).

Patients referred will be added to the TYA and Late Effects Clinic waiting list. 

If you believe your patient would benefit from our service, and they meet the criteria, please refer your patient via the form at the link below

Provide all the details requested in the form, so we can discuss the referral.

Inclusion criteria

  • Received a cancer diagnosis and anti-cancer treatment before 25th birthday
  • Currently aged over 18 years
  • Five years from end of treatment and completed oncological surveillance
  • Referral made to the TYA Late Effects Service by treating Consultant or Clinical Nurse Specialist including completed EoT summary

Alternatively, referrals will be accepted from GPs for patients treated historically and/or out of region.

Exclusion criteria

  • Received first cancer diagnosis after 25th birthday
  • Patients who have not received anti-cancer treatment
  • Patients currently under alternative oncological surveillance
