Referrals

Referral to this service can only be made by a GP or consultant - self-referral is not accepted.

Please consultant your doctor if you think you should be referred to this service.

GP referrals

All GP referrals must come through e-Referral.

Consultant referrals

To make a referral to the Oxford Sarcoma Service a clinician should send a clinical referral letter to sarcoma.referrals@nhs.net with copies of images sent from PACS via IEP.

No sarcoma opinion can be given without radiology review, pathology review, copies of surgical notes and oncological background.

Clinical referral proforma

Please note that patients will not undergo case discussion without these details.  If these are covered in a detailed referral letter, then this would be helpful, but it is essential we have as much information as possible.

Radiology review

All images should be provided (using PACS via IEP) with the referral letter.  A report should be enclosed.

Pathology review

Representative blocks and slides from all pathological material together with copies of radiology images on CD should be sent to:

Surgery review

If cases have had previous surgery, copies of the surgical notes should be provided. This is particularly required in cases of re-excision.

Oncology review

Previous chemotherapy (drugs and doses, number of cycles, toxicity) should be provided.

Previous radiotherapy (including photocopies of plans, prescribed doses, if indicated) should be provided.

Patient information

The proforma section on what information the patient has been told is mandatory.

International referrals

Indication of the source of funding for medical care would be very helpful in efficient planning of international referrals.

Clinical trials referral

An increasing number of clinical trials are available for patients with relapsed sarcoma. Please indicate whether your patient would wish to proceed with trial discussion.
