Referral information

Who can refer?

Referrals from outside the Trust must come from a consultant.

Other health professionals may refer in the first instance to:

  • Mr Tim Theologis, Consultant Orthopaedic Surgeon at the Nuffield Orthopaedic Centre

indicating that gait analysis is requested.

An outline of appropriate patients to refer to the gait laboratory can be found using the link below.

If you are unsure as to the suitability of your patient for gait analysis, please contact us and we will be happy offer advice.

How to refer

Please fill in the referral form, which can be found at the links below.

All grey fields are required information. Other fields may be left blank if referrers are unsure of what to fill in.

Please send the form to Oxford Gait Laboratory, along with a letter with relevant details about the patient.

We have a number of contracts with primary care organisations around the UK. If your patient is registered with a GP associated with one of these, we can arrange an appointment for them within 2-3 weeks.

If not, we will need to contact their primary care organisation and agree funding prior to arranging an appointment.
