Personal information / medical records

Under the Data Protection Act 1998 you have a right to be told whether the Oxford University Hospitals NHS Foundation Trust holds any information about you, and if so, to have that information disclosed.

Examples of information held are:

  • health records
  • employment records
  • letters.

To have information about you disclosed, please make a 'Subject Access Request'.

Copies of information can be sent to you, or you can request to view your information with a member of staff to explain any terminology. Sometimes, some or all of the information you ask for cannot be provided. If this happens, we will explain the reasons why.

Before processing your request we may need to check your identity. There is also a fee for requesting copies of your notes.

Charges for photocopies of records are:

  • up to 300 pages, £25 including postage
  • over 300 pages, £50 including postage
  • copies of X-rays, £10

If you only wish to view your notes there is no charge.
