Home Assessment Reablement Team (HART)

The Home Assessment Reablement Team is a community-based service to help people regain skills lost through ill health or surgery, which provides care across Oxfordshire to people over the age of 18.

The service comprises:

  • Reablement Support Workers
  • Team Leads
  • Coordinators
  • Assessors
  • Occupational Therapists
  • Physiotherapists.

HART is a free, short-term service for up to six weeks, and operates 7.00am to 10.00pm 365 days a year.

HART has three bases:

  • John Radcliffe Hospital (covering Oxford City)
  • Abingdon Health and Wellbeing Centre (covering South Oxfordshire)
  • Horton General Hospital (covering North and West Oxfordshire).

Opportunities

Reablement Support Worker

  • £21,210 p.a. pro rata
  • Starting annual leave entitlement 27 days pro rata, plus Bank Holidays

We are recruiting Reablement Support Workers to our City and South teams.

Reablement Support Workers help with personal care, such as washing, dressing, preparing meals and encouraging mobility through basic exercise.

Each service user has an individualised care plan, and Assessors and Therapists review this regularly.

Experience is not essential, although being a caring, supportive and sympathetic person is. We provide training for all our staff.

We can offer up to 36 contracted hours: all Reablement Support Workers have a set four-week rota which includes early and late shifts and weekends.

Reablement Support Workers must have a full UK driving licence and their own vehicle: mileage is paid at 45p per mile.

Reablement Support Workers must be computer literate and comfortable using smartphone apps on phones that are issued by the Trust.

We provide tunics and trousers for our Reablement Support Workers to wear while they are on a shift, and our staff are required to wear PPE when completing their visits.
