Referrals and enquiries about specific patients

This section concerns making contact about an individual patient and his or her problems, to:

  • refer to the services in the Centre
  • ask whether our services are appropriate
  • seek information and advice about the best way to help an individual patient.

Making a referral

The referral process varies from service to service. Please select the appropriate link from below:

Referrals by letter

Where a referral form is available, please complete that as it will request the information that we need to process your referral efficiently.

If sending a letter, there is a basic minimum of information required on any referral:

  • patient name, date of birth, usual home address and (if known) NHS number
  • general practitioner's name and address
  • the referrer’s contact details (telephone numbers, e-mail)
  • the patient’s current location if not at home
  • main underlying diagnosis (or presumed most likely diagnosis)
  • your primary request or expectation (one sentence is sufficient)

Additional information that is almost always very helpful includes:

  • current level of ability and what help and support is currently needed (this includes any essential equipment)
  • relevant contextual background information including recent history, social circumstances and accommodation
  • other services currently or recently involved
  • current medication

Timescales

We aim to review all referrals within seven days of arrival and then to respond to the referring person, both confirming receipt and indicating the likely outcome and timescale.
